Requirements vary by university, programme, citizenship and application location. Check the notice for your exact intake before submitting.
Who needs it and when
The requirement is most common for study over six months, scholarship applications and a study residence permit after arrival. Short programmes may use a different list. Do not take the exam too early: the six-month period needs to cover submission or registration.
What makes the form valid
Use the official template and ask the clinic to complete every listed examination. Common rejection reasons include blank fields, no doctor's conclusion, no hospital stamp, an unsealed photograph, or missing laboratory and X-ray attachments.
- Name and date of birth match the passport.
- The form is completed in Chinese or English.
- Photograph, doctor's signature and medical institution stamp are present.
- Attachments are legible and translated if the notice requires it.
Why a recheck may happen
For a residence permit, the local authority or university may send a student to an authorised centre to verify the overseas report or repeat the examination. This does not make the original form pointless: it may have been required for the application itself.
Handling medical records
Upload records only to the official university or scholarship system. Keep the original separately and do not send the full form in casual messages: it contains sensitive personal data.
